Referral to the SHARAP office is made after consultation with a specialist doctor, and receipt of a referral form for surgery.
We at the SHARAP offices understand the concerns and struggles involved in the surgical procedure you must undergo, and therefore our goal is to make it easier for you as much as possible and to help you arrange the administrative matters before the surgery. We will be happy to provide you with a comprehensive service with a patient, courteous and personal attitude.
SHARAP office services include:
- Providing information about the surgeons within the SHARAP framework
- Providing information to arrange a payment guarantee with the health funds and insurance companies.
- Eligibility check and settlement of payment guarantee of "Klelit Perfect" and "Shaban Maccabi" insureds.
- Assistance in submitting/arranging the forms to receive a payment guarantee in front of the insurance companies in the agreement with Shaare Zedek Medical Center:
"Harel", "Kelal", "Ayalon", "The Phoenix", "Menora", "Migdal", "Dekla".
*Our ambition and goal is to reach settlements with all insurance companies.
Most of the procedures are done through financing arrangements with the supplementary insurances of the health funds, private insurance companies and the patient's deductible.
Each SHABAN in kupat holim has its own regulations detailing eligibility within the SHABAN (additional health services): which surgeries/tests are covered, what is the scope of coverage, is there a need for a deductible, etc.

Please note: the cost estimate before the surgery is based on the operation planned in the surgery. The final cost will be determined according to the actual operation. Therefore, if there is a change (procedure/additional treatment, use of accessories, etc.), there may be a change in the amount of the payment accordingly.
Choosing a surgeon
You can choose a surgeon who is not in agreement with the health funds and insurance companies and arrange the payment privately which includes the costs: for the pre-operative clinic, the hospitalization cost and the SHARAP cost for the surgeon.
Before arriving at Shaare Zedek Medical Center, you must have:
- An ID card that includes a photo: If the patient is a minor, the attachment to the ID card of one of the parents must be presented in which the child's details are indicated, or a guardianship document depending on the circumstances.
- Payment methods: You can pay for the medical procedure in cash, check or credit card.
It is very important to have all the documents required for the day before the surgery, because if you are missing one or another document, the surgery/treatment may be postponed or delayed.
